Verify your Signature(s)

Witness shall sign at section "Witness' Signature" in the account opening form after witnessing your signing at section "Client Declaration" in the same form.

Certify your Proof of Citizenship and Identity

Witness shall certify cop(ies) of your official ID(s) by signing and stating "Certified True Copy" on them.

(An official IDs must be issued by a government body bearing the holder's photo. If you don't have any of the below, please check with us for other types of acceptable photo ID.)

Witness' Signature (Refer Account Opening Form: Part 42)

Arrange a Witness ^{(Note 1)} to:

sign at Part 42 of the account opening form after witnessing your signature(s) in Part 41; and
sign at Part 20 of the "Section A: Particulars of Key Person(s)" after witnessing the Specimen Signature in Part 19; and
certify all supporting documents as "True Copy".

Note 1: Witness to Certify all the Signature(s) and Supporting Documents: An acceptable witness can be a MONEX BOOM employee, Justice of Peace, a branch manager of a licensed bank, lawyer, certified public accountant (practicing), chartered secretary (practicing), notary public, or a SFC licensed or registered person accredited to another intermediary.

Before signing on this form, each of the signer(s) shall present the original of his / her Identity Card or Passport to the Witness who will certify his / her identity. All the signature(s) and the affixment of the Company Chop on this form shall be made before the Witness.
All the supporting documents (including Official ID copies of Key Peron(s) and Guarantor) to be submitted with this form shall be signed by the Witness and stated as "Certified True Copy".

Certified copy of Shareholding (Ownership) Structure^#

Company Registration

For the EACH of the compan(ies) under the organization structure: (including the Corporate Applicant, all intermediate holding company(ies) and the Ultimate Holding Company (if any))

Certified copy of Certificate of Incorporation ("CI")^#; and
Certified copy of Memorandum & Articles of Associations ("M&A")^#.

Board Resolution

Certified copies of the following board resolutions:

for approving opening a MONEX BOOM account; and
for appointment of Authorized Person(s) and signing arrangement.

Address Proof (Refer Account Opening Form: Part 16)

Photocopy of the bank account statement issued within 3 months.
(It should be printed with the Applicant's Name and Address)

Key Person's Information (Refer Account Opening Form: Part 20)

EACH of the Key Person(s) shall provide:

Completed "Section A: Particulars of Key Person(s)"; and
Certified copy of Key Person's Official ID ^{(Note 2)}; and
Photocopy of Key Person's Proof of Residential Address ^{(Note 3)}; and
Completed & signed "Key Person Tax Residency Self-certification".

Note 2: Official ID: Official ID shall be issued by a government body bearing the holder's photo.

Note 3: Proof of Residential Address: Proof of residential address can be a photocopy of utility bills, bank statements, etc. that was printed with the one's residential address and issued within 3 months. Kindly note that P.O. Box are not accepted.

Company and Directors Records - either (I) or (II):

(I)

For Hong Kong Incorporated Company:

Certified copy of Business Registration Certificate^#; and
Certified copy of latest Annual Return and subsequent form(s) filed to Hong Kong Company Registry^#.

(II)

For Non-Hong Kong Incorporated Company:

Certified copy of the Certificate of Incumbency or Registered Agents Certificate# (issued within 6 months); and
Certified copy of the Register of Directors^#; and
Certified copy of the Register of Members^#.

Guarantor's Information and Agreement (Refer Account Opening Form: Part 13)
(if applying corporate margin account)

The Guarantor shall provide:

Completed "Guarantor Information"; and
Signed "Guarantor Agreement"; and
Certified copy of Guarantor's Official ID ^{(Note 2)}; and
Photocopy of Guarantor's Proof of Residential Address ^{(Note 3)}.

W-9 Form (declare U.S. taxpayer status)

The W-9 Form is required to be completed by a U.S. Person (include U.S. entity, U.S. business or organization and U.S. domestic trust) to certify a Corporation's U.S. tax status. Under the Foreign Account Tax Compliance Act (FATCA), a foreign financial institution (such as MONEX BOOM) is required to report to the U.S. tax authority the financial assets in the Corporation's MONEX BOOM account. And it is MONEX BOOM's policy that U.S. stock trading service is not offered to any U.S. Person.
